About doctors' letters for Dignitas

Hello, I have a severe degenerative illness and will die from it. I am in extreme pain and want to end things early before ending up bedridden in a badly maintained NHS hospital. Does anyone know in the UK where I can get a doctor's letter for Dignitas? I would give the doctor my test results, they would examine me and I would pay them for the letter. Thank you if anyone knows where I can get a letter, from a UK doctor, that Dignitas will accept. ps I am very grateful as I have spent a lot of money and done a lot of things to try to get well for over 12 years and none of it has worked. Not the NHS, USA or Europe treatment and more. I do not think that there is anything wrong with going and dying when you have no family and your care would cost the government money and you are ill and alone. I have also tried to work and I cannot stop having flashbacks or cannot stand very long or similar. What I have is chemical, electricity and other sensitivities plus severe allergies plus the symptoms of flashbacks etc.
