Very dangerous cholesteatoma and candida

Hello,
I have been suffering from cholesteatoma since childhood, lost my hearing, now I am implanted with Cochlear Implant.Cholesteatoma is still affecting me, having surgery every 2 years.The last cholesteatom was very difficult to remove, 6 hours surgery, very life threatening,could develop brain abces, meningitis or coma. The last ENT doctor found candida in my ears before surgery. Put me on very strong medication.
Some time ago I tried a very low carb diet.Eliminated some white chunks( sorry for the image) but my joint pain disappeared.Since Science doesn't answers me anymore, I would like to ask for your help.Please recommend me some strong antifungals( ketoconazole?), the best pro/prebiotics and appropriate diet.
I also have chronic sinusitis, sinovitis (joint issues),chronic otitis, you name it.
