Probably ascaris/roundworm. The pic isn't too clear, but they do come out in pieces when killed and partially digested. Also, from your description of spagetti looking stuff.

Yes, you probably need to do many cleanses, and something stronger than Humaworm, or Humaworm in addition to the added things you already mentioned like aloe, garlic (lots of it), psyllium etc.
