It looks like you might be dyslexic. A lot fo people associate dyslexia with spelling problems and reversing some letters/numbers but there is much more to it.
Please read a book "The Gift of Dyslexia" by Ronald Davis . You might find it at a local library, or purchase it (try amazon.com) new or used - as far as I remember it costs less than $20.
What you mentioned about receiving instructions and doing totally opposite - is described, and explained in the book, as well as other problems that you are having.
The book also tells what to do to get it under control. It require some work, but it is easy (you might need other person help with it).
My son was diagnosted with severe dyslexia in multiple areas when he was 9 years old. We used the advice/therapy described in the book. It has helped him tremendously.
