I didn't realize, or perhaps had forgotten that it might go unrecognized, as you say, until a person attempts to fast.
I see why so many people are 'into' using ketone sticks to watch the shift over to ketosis. Not so much lately, but a few years back everyone was...

I recall when I first ran across this enzyme deficiency information, it rather spooked me, and I haven't wanted to bring it up very often, such as when people complain of weakness, or what sounds like 'common' difficulty with fasting--because I don't want to scare people off water fasting!!!

There is enough pressure out there in the world, put on water fasters. Well-meaning people of all sorts think going without food will kill or seriosuly harm a person. Time was, I believed the same, of say a 29 day water fast.

Instead of being alarmist, I always want to encourage, as you do, people to read the literature. Go back to the basics, of what's been written long ago, and is offered freely, on the Soil and Health Library. I don't know how many times I've repeated that name, but I'll keep doing it. :-)
