1) It was impossible to leave my lips alone because they would get so hard and dry that they would crack and bleed when I would attempt to smile. Still, it did happen a few times that I didn't have access to chapstick for about half a day. I would say that my lips turned into a combination of what you described: they would get really hard, but the skin wouldn't "pile up" - after they hardened, they would fall off in little flakes. At their worst, this hardening and peeling extended past the vermilion line and was headed to both my nose and chin!!
2) I never bit or licked my lips in my life.
3) The first first time the problem ever presented itself (when I was able to heal it for a year or so by using hydrocortisone 1% for 2 weeks), it started as a small patch in the middle of my top lip. When it came back with a vengeance a year later, it came by completely covering only my bottom lip. A few months into the problem, it eventually spread to my top lip. A few months later, it started passing the lips by a little bit.
Sometimes I would feel my lips worsening at an accelerated rate. When this would happen, I would begin to feel a "tingly sensation" at the bottom right of my lip. When I'd look in the mirror, a small circular part would be really red and swollen, almost as if there is a gland there.
