I won't go into the whole lengthy explanation of it but I'll just add to what has already been said.

I'm not a doctor or nutritional expert so I can't give you a very technical explanation. I can however tell you how I understand it.
There is a distinct difference between fasting and starving. Fasting is a cleaning and healing process while starving is a destructive process that will come after fasting if the fast is not ended.

When the fast starts hunger usually goes away after a day or two. This is when the body "feels" whats going on, the absence of food, and starts it's cleaning and healing process. At the end of the fast, when the body has done it's cleaning, it will tell you by hunger returning. This is the primary indicator that the fast is to be ended. If this is ignored then the starvation process will probably begin.
