Hey,
Thanks for this post, and I also had a breeze through your poll on SD, which is really interesting.
I have had SD for 3 years now. Like everyone who has it, I want to get rid of it, because it sucks.
I have come to this conclusion. SD is caused by the mind. You should have asked the question in your survey, "did your SD come at a time of personal trauma?"
It is also made worse by anxiety, and stress. There have been times, when I have used no creams or anything, and my SD has vanished. Why? Because I suppose I was relaxed. This was on holiday once, when I could forget about everything else in the world.
There have been times, where I have used hydrocortisone and it has done absolutely nothing. Why? Because I was stressed.
The trick to getting over SD, is to eliminate the causes of stress in your life. If you can't do this, learn to not be affected so much by these causes.
For example, when I'm at home, chilled, watching a movie, on my own, or with my GF, then my SD doesn't affect me. When I'm out with friends though, it can sometimes become worse, depending on who I'm with though.
It is external factors that cause SD, so it's important to be in good environments, to do excercise, and more importantly, do something you love. All of this rids you of stress.
Hope this is an insight for you, and that somehow you can incorporate the impact of stress/anxiety/personal trauma on SD in your poll. Also, my SD has often become worse when I have had a fight with my GF, or a stressful encounter with the parents.
