Why was it necessary for the IEC to come up with the 60601 Standard for power adaptors designed for instruments that apply energy to humans?

I recently received back a zapper from someone who used it with a wall wart. They were probably saved by the fact that the 25 volt 22 mfd electrolytic blew. And I mean completely vaporized, including much of the traces on the PCB.
