Is she taking any blood thinners or cardiac glycosides such as digoxin or digitalis? If so, she should start with very small amounts of oleander and gradually work her way up, perhaps reducing her other medications a bit to compensate. However, it should be pointed out that an oleander capsule contains only about 1/40th to 1/30th of the amount of cardiac glycosides as does a typical dose of digoxin/digitalis.
Two capsules twice a day is quite a big dose of oleander to start out with. It is recommended to start out much smaller and work your way up as the body becomes acclimated. Personally I would recommend starting with one capsule per day and then increasing to two, then three, etc. The maximum dose, if tolerated, for a 150 pound adult is figured to be about 8 capsules of Sutherlandia OPC (the old formula with 80 mg of oleander) or perhaps 6 capsules of the Rose Laurel OPC or the new Sutherlandia formulation, both of which have 100 mg of oleander per capsule.
I note that prior to the advent of oleander supplements, it was recommended that a person who made their own oleander extract according to the directions for "oleander soup", should start out with only 1/4 to 1/2 teaspoon per day and work up to one tablespoon three times a day.
