This sounds more like an allergic response.  Most of the symptoms are typical allergic symptoms. The cold feeling can occur from the epinephrine released during allergic responses that constricts blood vessels.  A rash related to allergies would likely be eczema, which is in large part an allergic reaction.  The rest being problems with liver toxicity.

I recommend he really tries to build up his adrenals first and cleanses the liver with bitters.  Wheat allergies are actually pretty common.

As for the itching topical steroid creams will not do a thing.   Nervines such as periwinkle or an inhibitory amino acid such as taurine, glycine or GABA would be a better choice.
