I don't know any docs that do email consults but I know of a very good medical organisation called ACAM (American College for Advancement in Medicine) - they have two doctors based in Italy and although I don't know anything about these doctors, I think the fact that they are members of ACAM says something really good to me, they should be open to natural methods and non-drug therapies - they also have these docs emails listed. The web link is below.
As for the NAC, you could probably buy some on the internet and have it mailed to you, should be okay with customs etc, as it's just an amino acid. In the study of PCOS women, I believe they used 1800mg a day for six weeks - I got great results with just 600mg a day. Another natural product I've read that is good for PCOS is Inositol. Check out the cleanses on CureZone too - I think they may be very worthwhile.
