If a root canal has failed, its actually better to have the tooth removed and replaced with an implant. Implants have a much better prognosis than a second root canal, and they last a lifetime.
Cost is completely dependant on location. However, I would expect around 5,000 in private practice. My suggestion would be to find a dental school nearby. Many of them have implant programs where an undergraduate dentist would plan the implant under supervision, a lisenced dentist would place the implant, and the undergrad would finish it. The price is usually 50% or less than private practice.
