Please help with fever occuring after 7 weeks of treatment

I wish all of you a great year especially health wise.
A family member was first diagnosed with Inflammatory Bowel Disease, and started using steroids for 6 months. However, no improvement had been seen, so doctor advised surgery due to blockage. After operation, the illness got worse, with a lot of coughs and very very high fever, and finally was diagnosed with miliary TB, seen on lungs, periton, liver, ilium.
The treatment with INH, RIF, ETB, and PZA started 7 weeks ago. At first fever went away, then it came back. Typically is starts every late afternoon, then has its peak (37.5-38.0 degree Celcius) around 11-12 pm, then starts getting lower. However, in the last 5-6 days, the patient started taking pills for high fever, around 38.5 degree celcius, occurring at similar hours, with chills.
We have consulted many doctors including lung specialist, infection specialist, internal disease specialist. They have checked almost everything, including frequent blood tests and frequent x-rays. On the tests, the results are getting better day by day. The clinical situation of the patient is as well getting better, with weight and energy gains. However, the reason of fever is still not answered.
It has been 7 weeks since the treatment has started, and all the doctors mention that the fever should have been disappeared by now.
Can anyone please comment on this situation? I understand that miliary TB is not common, which means it is really hard to find experienced doctors with the treatment of miliary TB. We really feel very stressed with the situation of ongoing fever. Awaiting your kind assistance and comments.
