2 year boy with major Constipation

My cousin’s 2 year boy has major Constipation and it’s getting worse. This problem started almost at birth. His older daughter had the same problem but “grew” out if it by the age of one. His 2 yr old Son does not pass a stool in 4-5 days and when he does it’s very painful because the stool is hard and almost the size of a fastball.
Doctors have given him a barium enema and many other tests – all normal. They are at wit's end over this problem. They’ve tried giving him more fruit juices, changed diet, suppositories, etc – but the condition continues to get worse.

Any comments you can offer would be appreciated.
