need help with OBESE DIABETIC friend

We have a family friend.
He is 60 years old, probably weighs about 350-200 pounds, has diabetis (he doesn't need shots).
He only has 1-2 BMs a week (if not less).
So he is not in good shape. He recently said that part of his foot had dark skin, he peeled it off, and it was dead under skin.

I want to help this guy get better. But I don't know where to start. He is a Construction contractor (always works).
What would be the best ways for him to get healed, cleansed, etc...?
Liver cleanse, then parasite cleanse? What do you recommend?
Since he is obese, he'd probably need a gentler cleanse, right?
