I completed your questionnaire. It did not take 3 minutes, but more than half an hour! The vaccination questions are incomplete: I once had a vaccination shot for cholera, which gave me temporary coma after about an hour of the vaccination. The questionnaire did not provide space for including it.
Your questions about health problems are on an excessively long list, which makes the answers more and more imprecise the further one proceeds with the questionnaire, as one does not remember which answers one ticked, and one tends to read through the whole list with impatience and less attention. On the other hand some symptoms are not imagined nor included in the list and it is not so easy to insert them.
You also need to correct some typos in your questions.

Your idea of making surveys about various topics and health problems ix EXCELLENT. I hope this will provide a basis for studies-articles about "alternative" therapies and health care practices, as we do need to have materials appear on Pubmed!
I will send you some suggestions about surveys to make, especially about health problems in connection with dental practices, such as: a study of people with specific illnesses (e.g. M.S.) and the kinds of dental prorocedures they have had in their mouth. People with, say, throat cancer, and previous dental treatments, etcetera.
Thank you very much indeed for thinking up these studies. They are long, long needed, and welcome.
