The time differences, I believe, have to do with the make up of the "bad things." I find that there are different contact times listed with water treatment as well.
For example:
I use a MIOX pen to purify water while out in the wilderness. Here is the specification sheet.

If you look at the contact time required for the different "bad things," you find that they suggest that viruses and bacteria can be eliminated in 15 minutes, and Giardia takes 30 minutes, however it takes 4 hours to kill off Cryptosporidia.
I called MIOX to inquire about the test strips that are provided with the unit. I also asked about speeding up the process in emergency conditions. I suggested that by doubling or tripling up the dose, I may be able to have water available in a shorter time period. My concern was with the higher dose of the MIOX chemicals. Their chemist replied that the best bet was to rely on the test strips to get the correct ratio. Even if you quadrupled the dose, it would still take 4 hours to kill off the Cryptosporidia. They have a very thick membrane, and it takes time to penetrate it.
It is possible that the same thing is going on inside our bodies. Malaria may be "thin skinned" and easily accessible. Other "bad things" may be buried deeper, or need longer contact time to eliminate.
I am not sure if this is the way it actually works, but that is my "two drops" worth.
