My answer is based on common sense (and more than 20 flushes behind me). I don’t have any medical training. You can probably say the same.
If it was my personal decision, I would do a LF, but with a great care.
LF cleans your liver and there is no direct effect to your kidneys.
But cleaner liver will aid all the other organs in our body to function better.
So, the help for the kidneys needs to come within the body they belong, to make them functioning better.
If that works there is hope, if not, the only option is transplant as you already mentioned.

When I say a flush with great care, I mean to start with reduced amounts of ingredients.
Recovery should be monitored also carefully to see if improvements happen.
