Well, I've got the EXACT same problem. I've experimented on myself till I'm blue. But I'm still itchy and have those same welts that appear after scratching as you describe. It's been going on for me 4 months now. It's not so bad in the daytime nowdays...but at night the itching/scratching gets rather silly. Luckily, I seem to be very slowly improving. I used to depend on Zyrtec....but now I seem to get along ok without it.
Here's what really helps me:
Instead of scratching with your fingernails, scratch vigorously with a natural bristled dry brush. Amazingly it not only physically satisfies the itch (unlike fingernails) but it also quickly eliminates the underlying irritaion in that area due to the increased circultion. Try it!
Also..you mentioned cayenne pepper. This is an incredible secret: consuming cayenne. I found that if I mix several shakes of cayenne pepper in a small glass of vegetable juice (or warm water) it stops the itching for a good 2 hours. It's been a God-send to me. Cayenne pepper is great for alot of things, so don't be shy about using it. I take it almost everywhere I go. Most people carry a wallet or purse...I carry cayenne pepper! My wife thinks I'm a certified nut!
I also started seeing improvement after doing enemas. This is not a quick fix, but seems to help in the long run. Long, hot baths also help.
Scaling back on your anti-fungals will also help. I choose to go slowly nowdays. It's not a race. Good luck. Tom
