Lots of things come to mind, such as liver and other cleansing (including coffee enemas to help open up the bile ducts) and iodine (including perhaps painting iodine on the scrotal area - see: http://www.marksdailyapple.com/forum/thread45205-249.html#post836849).

It would be helpful if you post what other pills besides Tramadol you're on and what supplements (and amounts) you're taking.
About 1) heart arrythmias are often due to electrolyte imbalance, because I have thyroid and adrenal problems I often get them but have been able to successfully get rid of them on different occasions with either minerals or natural remedies, please go to www.earthclinic.com/CURES/heart_palpitations.html and in particular read the post by "Ted from Bangkok".
2) Floaters are often caused my fungus/candida, if you've had any mold in the house or been exposed to it at work you may want to look into that.
3) Perhaps exercise hurts because of the lactic acid, just a guess though! Have you tried drinking more water? Dr. Batmanghelidj, author of the book "Your Body's Many Cries for Water" said most kinds of pain respond well to an increase in our water intake and being a heavy guy you should be drinking aprox. 150 oz. of water daily.
4) My liver enzymes are also high but cause has been undetermined as yet. My own guess is that I may have gallbladder stones as I have felt discomfort in the liver area, especially after a large meal. After some research I opted for taking milk thistle and was going to order alpha lipoic acid but opted for this instead (but I haven't started yet...). I also made changes in my diet and started buying organic veggies and some grass fed beef and now use much less butter than before. And between that, taking digestive enzymes that include ox bile, the milk thistle and applying castor oil packs , usually overnight, I haven't had the bothersome "ache" in my liver in at least the past 2 weeks (maybe longer), so give them a try, it's the most helpful uninvasive therapy that I know of. Ah, and get lecithin granules, it helps heal a fatty liver.
5) The above iodine suggestion may be good. Also, if you're not taking zinc or MSM (for the sulphur), try those, I reead they are important for testicular health. I think you could even have a separate cloth drenched in castor oil to do the packs there too.
At this point any junk or fast food should be completely out. And if by chance your body doesn't handle wheat/gluten and dairy or even some other foods well (allergy), it could cause a lot of things to go wrong. Wheat (even whole grain), for example, is notorious of creating anti-bodies in sensitive people (like 'moi') which then attack organs like the thyroid, adrenals, liver, etc. Despite knowing that my father had celiac, which is or can be hereditary, and over time seeing my body react negatively to those foods, I never really wanted to give up my favorite foods for good and I played a "little game" of going off them until I felt better, then make "a few exceptions" which were never that "few", well, that surely backfired on me big time and it took my getting scared out of my mind with some recent "attacks" to finally learn my lesson. And I want to tell everyone who hasn't explored this type of food intolerance or has even suspected it but is not convinced, that it's not worth it to stay in denial, we can self-test for free by simply withdrawing those foods from our diet for at least a week, sometimes it could take longer, I remember when I first stopped eating cheese and yogurt my nasal congestion got worst at first, but then after 1 week or so it got better.
I also recommend that you read up on the Blood Type diet, I've found the recommendations for my blood type (O) to be pretty on target (wheat or dairy are "avoids" at least for Os & Bs), and as we see all the time what works for some doesn't work at all for others so the key could very well be in our different blood types.

You've obviously got a lot going on but what caught my attention is your migraines. I found that certain things could bring on my migraines which began when I was ten years old. Being around open oil or gasoline could do it, eating pancakes and syrup (yup, and I love 'em) and other things too. Along with them I would also get flashing lights. Aspirin rarely even helped and they would often go on for days at a time. I know this is kind of old but have you heard of Dr. Mona Lisa Schulz? She is a medical intuitive who does remote consultations by phone; she has a radio show on http://hayhouseradio.com, and years ago when I listened regularly I called and was able to talk to her which was a remarkable experience. I've come to believe that all chronic illness has an emotional or spiritual component and unless we "clean" that up we might try all the "miracle" cures out there and at best just improve a little but never totally get cured, and very often the reason something from the outside works is because it rebalances our vibrational energy, that is more likely to happen also after the original cause for the illness no longer exists (like someone who developed an illness because of a job they hated and eventually left, or living with someone they couldn't get along with and either of the two moved away).
