The support nutrients will be important to you. You might already be on some of them. Selenomethionine 200 mcg, magnesium 400-800 mg, vit C 2-4 g, B complex 50-100 mg. Drink at least the amount of water in ounces equal to half your body weight in pounds. You can add 1/4-1 teaspoon of salt to that -- drink throughout the day or split into two doses. Or, instead some people prefer the salt push when they're feeling very bad. It helps bind the bromine so that your kidneys can clear it out.

Certain iodine-literate doctors feel the bromide excreted by iodine therapy can be eliminated
more efficiently by the chloride present in mineral-rich unprocessed sea salt. The salt is added
to water in a specific way. Eating salty food is not effective. Table salt is only used in an
emergency.
Many iodine takers found the Salt Loading Protocol devised by Dr. William Shevin (and
presented at the February '07 Iodine Conference) to be effective in eliminating the side effects
such as bromide sedation, acne, brain fog, brassy taste, mouth sores, frontal headache or other
symptoms which occur in a small percentage of patients.
Everybody has their own timeline of detox, so unfortunately it's hard to know what you may expect. I had difficulty at low doses, and thought I was poisoning myself. My version was vicious headaches. After I found this site, I discovered I wasn't poisoning myself, I was in detox. That was a relief. Then I was able to stay on it, and drink my water and take my supplements, and got past the headache stage. I kept increasing my dose, doing okay between 50-100 mg, then at 150 mg Iodoral I landed in a black depression I was very familiar with. I cut the Iodoral in half and added an equivalent amount of SSKI, and that gave me relief. Ever since, I've used a combo of Iodoral and SSKI to best effect.
Your detox may take on its own personality. You may have pains in your body that revisit sites of old injuries. Three times my husband's smallpox vaccination scar acted up. And my last tetanus shot site hurt so much I thought I'd just been injected. Bromine will come out your skin. You may have bad breath, foul armpits, stinky sweat. As you can become braver, take more iodine. You might investigate buying SSKI or making your own to partner with the Iodoral. SSKI may be a little gentler on your system, and you can raise faster. As you get to 100 mg, you'll probably get over the hump, and detox will just be noticed, not knock you over the head.
I don't know if this is encouraging, or discouraging :) But detox is not endless, that I know. If you get overwhelmed, stop for a few days, a week, whatever you need. Keep drinking and sweating, and slowly you'll gain ground.
I have heard of some people taking 2-3 grams of niacin, either instant-release or slow-release, such as Slo-Niacin, but "several grams" makes it sound like you take even more. I can tell you're being careful in your approach, so I hope you're also doing regular checks of your liver enzymes to be sure the niacin isn't negatively affecting liver function. With such high cholesterol, optimization of your thyroid function is called for, then your intake of niacin could be more moderate.
