It's hard to imagine that your doctor is recommending a fast but perhaps stranger things have happened. IMO, it is the best way of regaining health but it's benefits will only truly be realised if you remove the cause(s) of your poor health permanently!
If the causes are specifically diet-related, then of course a fast will show plenty of improvements. If however, the cause is more to do with the other factors of health, you might still some improvement during the fast but only if you take full advantage of the time to relax and unwind.
The key health factors include fresh air, pure water, a wholesome, species-correct diet, exercise, rest and sleep, relaxation, mental poise, satisfying work/employment/hobbies (or in other words, a purpose in life,) and generally a low-stress, health supporting environment and lifestyle.
There's plenty to reflect on during your fast and no doubt you'll find plenty of time to do that reflection. You will also find great mental clarity to help you make good decisions about your future and how you can make healthful living part of it.
