I agree with some of the suggestions above, but as far as omitting meat that would depend on your child's specific needs, some of us do better on a higher protein diet and others do better on a lower protein diet. If his adrenals are low ie he is sluggish & tired I would urge you NOT to decrease his protein intake!
Yes, give him more water & veggies, but children can be difficult when it comes to eating healthy, especially if they already have bad dietary habits, so you may want to add some enzyme & probiotic support (for probiotics pearls are very tiny-easy to swallow and work well-get the generic brand to save on cost).
As for constipation, you may want to try a laxitive herbal tea, they have flavors that children would like. Give it to him when needed, but it is not something he should come to depend on...the idea is to get his system moving & working correctly.
The enzymes & probiotics will greatly improve his breath, but it will take time. Once his digestive system starts functioning better his breath will clear up.
If you are not supplying him a daily multi vit/min supplement add that on as well.
