I don't know if this would still be relevant as you posted so long ago. But anyway just wanted to share my story of dealing with PMS. It used to be unbearable, now it's almost unnoticeable.

Your symptoms sound very familiar. I think the most difficult was to actually connect everything, like how would anxiety, skin problems, dairy intolerance and fatigue be connected. I tried to cleanse, I did colon cleanse, and liver cleanse and parasite cleanse, was a vegetarian, and etc. It helped me greatly, but it didn't solve my PMS puzzle... until I started GAPS diet, restored my gut flora and then started supplementing with iodine. Then, it kind of all worked.

So, OCD and anxiety, well, as basically all the issues that you mentioned, are manifestation of severe gut dysbiosis and are digestive disorders. Please, look up Dr. Natasha Campbell-McBride, her "Gut and Psychology Syndrome" theory and her book, and the therapeutic diet.

You mentioned that you followed a low-fat diet with soy and some grains. Deficiencies in fat can basically starve you and your brain (as brain tissue is mainly fat). Low fat diet is a sure way to fatigue and Depression (speaking from very bitter experience...) Soy depletes us of vital minerals (Zn, Mg, Ca, etc) and disrupts our thyroid, hence PMS. Soy can ONLY be eaten in the form of organic fermented product and in small amounts.

you mentioned root canal.
I believe root canals is a consequence or a gut dysbiosis, ( root canals happened twice in my life - I had a course of Antibiotics and couple months later - a root canal...).
Both meds (especially long term, like you mentioned) or a course of Antibiotics can have devastating effect on gut flora and as a consequence - root canals, poor digestion, psychological issues, fatigue, etc

So, IMHO, you might benefit greatly from not just a cleanse, but a nourishing cleanse, something GAPS diet can offer. It was a major step in my recovery and I would recommend it to anybody with issues, like depression, fatigue, anxiety, OCD.

Besides GAPS diet, Liver Cleanses made a huge difference. Please, note, that liver cleanse is advisable only after colon cleanse. I kind of have a feeling, that the whole PMS thing is a sign of the liver that needs to be cleansed. The key to Liver Cleansing was taking Epsom Salt + cascara sagrada + lemon juice (about twice a month) and eating lots of red beets and apples. You may find something else that works for you.

You mentioned you were researching about the parasite cleanse. Please, note that some herbal formulas contain pretty strong ingredients, like Wormwood . If it is natural it doesn't mean it's safe!
Wormwood is extremely potent and Ayurvedically speaking EXTREMELY harsh on your Vata dosha, which in plain English, may cause severe anxiety, panic attacks, sleeplessness and even hallucinations (as it contains thujone). I personally had the most awful experience with Wormwood (until I learned better) and will not recommend it to anyone without a supervision of a professional herbalist! There are much safer herbs for parasite cleanse that do not have to make you nearly insane :) So, as much as I respect Hulda Clark , I believe recommending Wormwood is just not safe, and it is not for everyone!
There are plenty of herbs that can be incorporated into a parasite cleanse:

Neem Leaf tea - has been used in India for thousands of years, it works as a vermifuge (and has tons of other uses) and it doesn't make you go crazy :)

Hops tea - will make you calm and happy, will give you super restful sleep and will help you get rid of possible parasites, AND is a super herb for PMS!!!! it just takes away all the irritability, mean moods, and edginess!

Cloves tea - awesome stuff for parasite cleanse! kills the critters and their eggs, AND is great to improve digestion and to bring on menses.

Ginger tea, or just ginger as a spice - great stuff for an all over cleanse and for digestion and definitely a must for PMS - practically eradicates depression, kills fatigue instantly, brings on menstruation, strengthens your digestive power, eliminates headache and nausea, and simply makes everything better!

Turmeric root - has a massive cleaning power on your GI, liver, blood.

Thyme and Oregano tea - great for parasite cleanse and for PMS!

Cascara Sagrada tea - works as a vermifuge, as a non-habit-forming laxative, and as a liver cleanser.

Dong Quai capsules - I basically got back my regular menses thanks to this wonder herb, it's amazing what it does - it cleanses your liver and GI, it fights Depression and normalizes your hormones.

Black cohosh capsules - another wonder herb and made me actually a much nicer person before my menses :) (so, it's not just for menopause)

Cayenne pepper and lemon juice in water with honey! - super nourishing and cleansing drink for PMS and cleansing - helps to get rid of extra mucus, fight Depression and energizes!

Red beets do wonders for cleansing GI, liver and helping get rid of depression.

Iodine supplement (in the form of kelp or any other sea vegetables) or in the form of Lugol's Iodine - is a MUST for all sorts of issues with digestion, PMS and a general help. Iodine brought life back into me and I believe it is vital for anyone, especially for those who have heavy metal toxicity, candida (which is a consequence of heavy metals), parasites, chronic infections, headaches, PMS, fatigue, depression, etc

Magnesium (as a glycinate, malate or taurate) need to go with Iodine as well as

Selenium (can also be eaten as fish or mushrooms)

B-complex vitamins- is a must for PMS, IMHO

Zinc (in proper ratio with Copper, for example in the form of nuts or oysters)
