When it comes to a dog, I think the easiest way to do the castor oil pack will be as follows:
Materials needed:
Cold pressed castor oil
cotton/old tea shirt/towel
Cling film/plastic bag
Hot water bottle (lots of places recommend a heating pad, but it's really not necessary)
Method:
Soak a flannel/cotton cloth in castor oil and apply to the whole area of Lucky's liver AND abdomen. Wrap that (to keep it on) with cling film (maybe you call it plastic food wrap in the US), then apply another towel and place the hot water bottle to have it drive the castor oil into the body. You want to leave the pack in place ideally for an hour (longer is fine). I guess you're going to have to have lucky on her back or side, and may have to hold the hot water bottle in place (you'll have to be creative and find a method that's easiest/most comfortable for you both). After the hour, you can remove the pack. It's not a problem to leave the oil on her (if it's still oily) but if you prefer to remove it, wash her down with some water and baking soda mix.
(A great tip to drive the castor oil in even deeper, is to do a mix of castor oil and willards water. The willards water turns the castor oil into a creamy substance and makes for fantastic absobtion - This is the method I like best and have found most useful). You can get Willards water here:

You ideally want to do this for 3-4 days running and 3-4 days off. It's a very powerful detox therapy that also feels soothing. On the non-castor oil pack days, get some good cold pressed olive oil and massage around her abdomen and liver area - this will replenish and start building the liver and help with bile flow (essential for detoxification).
Don't be alarmed if her stools change with this, because the castor oil packs help rid the liver and colon of toxic waste.

By the way, Milk thistle is more of a liver rebuilder. But before rebuilding, it's best to detoxify the liver, so the castor oil packs will allow the Milk thistle to be more effective.
