I know that with cysts caused by hormone imbalances (pcos) that thy are formed from matured eggs not being released. They then die and become a cyst. With pcos the problem is whether the signal for the egg to be released comes or not, so it's a factor of ovulation not going through the full cycle. I'm not sure at all if the two are related, but that's one side of the story at any rate.
