I think those of us who have gone to many lengths trying to eradicate bad breath and who have hygiene regimens which are astoundingly high need to accept that the cause of our breath will NOT be cured through even more hygiene. Clearly, if your dentist has given your oral cavity a clean bill of health then attention should shift.
If the odor is obvious to others but not you and only becomes obvious to you if/when near air/conditioning or when you leave a room and return shortly after then I believe this is likely via alveolar breath, ie from the lungs. If the odor is like sewer/drain/cabbage/rotten egg/mothball/fecal I believe there is a deeper issue going on and it is likely that this is high levels of dimethyl sulfide which enter the blood stream and leave via expiration.
I am afraid I don't have the answers and I am reading around this subject myself but I would encourage those of you whose halitosis is NOT due to poor hygiene to consider this as the factor behind your condition and try to see if your physician/consultant can help. There have been Medical Research articles on this.
This may also account for those individuals who have symptoms akin to TMAU, but who test negatively for the condition.
