One possibility is that you are suffering from trichomonas infection, which is a protozoal infection. This can cause a fish smell and is often missed by standard testing by microscopic examination. Culturing is a little more accurate, but can still overlook infections.
And since this infection also infects the female organs even killing the protozoa in the urinary tract does not mean the infection will be gone. The infection can easily re-infect the urinary tract since the protozoa are still present outside the urinary tract.
My recommendation would be to talk to your doctor about getting treated for trichomonas just in case since this is the most likely culprit.
