Get a neti-pot from Mountain Rose or elsewhere and boil some water or use boiled distilled water, bring to room temp and flush the nostrils out with a capful of hydrogen peroxide and water. Do this many times a day.
Pour some peroxide in the ear and let it foam out every other day.

Also you can use table salt and baking soda in the neti or salt and Iodine and flush the sinus cavity if the peroxide is stopping you up.
This procedure will kill the worms.
Another way is to boil water and trap the steam and sniff up the steam to kill them as well like a facial steamer.
It will take months to get rid of most of them and hopefully all.
Inside the head or near the scalp I am not sure what to do there except take mega doses of Wormwood .
