I say "you should go" like it will be the easiest thing you have ever done. It won't. In fact, it will probably be one of the hardest things you will ever do.
It sounds like you really love this guy, but it sounds like this guy doesn't have a clue to the definition of love. Sadly, there's a lot of people in the world just like him.
Can he change? Yes, he can. Will he change? No, I doubt he ever will.
Don't try to "fix" him. You can't. Just look at ways to make yourself content while you are still in this situation. If you choose to leave, then work on making yourself happy and figure out why you were attracted to him in the first place. I say that because I was once in your place. No, he wasn't in a band, but we had a lot of the same issues. It took me 18 years, but I left. But, I also had to figure out why I was attracted to him in the first place. Back then, there was a movie star, James Dean, that I absolutely adored, so every man I saw that was similiar to him, I wanted. Back then, I loved those fast driving, cigarettes rolled up in the t-shirt sleeve kind of guy. In other words, I love a kick ass guy and I am past 55 now. I have matured and my 2nd husband is a little more laid back. But, I promise you, when I see Tony Soprano on TV, he gives me a thrill. Ha
I will be thinking of you and will say a prayer for you today at church.
