Diarrhea after gallbladder removal is due to constant trickling of bile into the small intestine.
This also creates a change in pH and therefore a change in flora (now prone for other species to take over).
Weight gain is likely due to efficient carbohydrate digestion with lack of fat digestion stemming from gallbladder removal (very common occurrence).
Hiatal hernia may indicate that food is sitting too long in the stomach due to change in the environment downstream.
I would say the primary focus would be to improve fat digestion as much as possible.
For this I would suggest bile acids supplementation with fatty meals (like cholacol by standard process).
I would highly recommend intaking essential fatty acids in micelled form - in this form they are water soluble and don't require bile to be emulsified and absorbed.
I'm having great results with Pharmax EFAs (I have my gallbladder but I have steatorrhea likely due to improper bile flow).
With proper levels of fatty acids you will likely loose weight (it affects thyroid regulation and hormone production) and will lower the risk of stone formation (by slowing cholesterol precipitation.
You may also want to look at improve bile flow by:
-Thinning bile (beet greens will do this)
-Slowly dissolve any accumulated stones (phosphoric acid, peppermint oil, GCG, chanca piedra)
- Liver Flush (would help if in conjuction with 2 above)
- herbal treatment to stimulate release and feed liver (I like Medcaps DPO from Xymogen).
Since you no longer have an organ to concentrate the bile, you may want to change your diet to have more & smaller meals with portioned amounts of fat.
Digestive enzymes are essential, not only for digestion (lipase in particular) but to discourage pathogen overgrowth (like cellulase).
I like multizyme and lact-enz by standard process
