Have you ever been adjusted and had your neck squared away by a good chiropractor? Ear infections respond great to chiropractic and if you had a bunch of them as a kid it would be worth your while now to get your neck checked out to see if there is any residual damage that is affecting your hearing. It might not get rid of all of it but it's worth the effort to find out. You know, get it down to a dull roar.
