You may have to adapt one from others you are able to gather from other professionals in related fields, acupuncture, chelation, even metaphysical healers like Reiki. Or do a google search for disclaimer for alternative medicine practitioner. I worked in a metaphysical healing clinic in Los Angeles that did aura balancing healing and every client had to sign one. We were also instructed not to give advice like "Take this, take that" but to say "If it were me, I would take(or do) this."
