Wow. Well, first of all I want to be clear that I am not providing medical advice. The complexity of your issues does surprise me.

Whenever I have dealt with allergy related topics my response is pretty much the same. Remove all "new" foods from your diet and get as much cardio exercise as you can. After a month or more without "new" foods I would hope that you would be mostly symptom free. You could then experiment with adding back the occasional "new" good one at a time to see what causes the reaction.

By "new", I mean foods that are relatively new to humans. While the major culprits here are milk, wheat and processed foods it is probably easier to suggest that you stick with "old" goods -- that is foods that we have consumed for over 15000 years.

This would include lean meats (ostrich, buffalo, fish etc.) and fruits and veg (although lay off any that you know you react to -- such as bananas.
