Something like a threesome goes through many couples minds, mine including. It's kinda hard to give you advice, you love eachother, you've acknoledged whats happened and are hopefully on the way to working it out, although i don't know if her moving out is the best thing, it seems a bit extreme, altough it might be nice to let you both miss eachother, when i have a thing with my hubby and leave i see how much i miss him and viceversa.
You need to know exactly what she's thinking and how she's feeling, don't let her drift away, i know women i am one, she probably needs complete reassurance that you adore her and still find her irrestitable, this has probably knocked her confidence down quite a bit. She's probably thinking that she isn't enough for you physically, i know my husband loves to look at me and be with me sexually but i still need to hear it from him, i wanna know how much i turn him on.
I'm just guessing but eveytime you get intimate she's probably thinking about that night and the only thing that will make it go away it time, she's not gonna forget that feeling but just let her know you'll be there when she does.
