Where do I meet girls?
at college. trust me, there will be a lot of them. walk around, go to house parties, join clubs. you'll find one (or a hundred) you like, and i'm willing to bet a lot of girls will be looking at you too. no worries.

How do I approach the girl?
do you have any female friends? this is a good start because if you know how to be comfortable with girls outside of sexual/relationship scenarios, it will make things easier for you when you want to approach a girl you are attracted to. be friendly, act like you would towards a new friend. stay calm, as long as you are respectful and polite even those weirder moments will come out ok. see the movie "say anything". john cusack's character's offbeat but completely dateable.

What kind of conversation do I start/how do I start it?
things that interest you. don't pretend to know more about a certain subject to impress her, but it's always attractive when a guy or girl talks passionately about something that interests them. that being said, if she's talkative listen a lot. listening is an underrated skill.

When is it okay to ask for phone number/e-mail address/screen name/whatever?
yes. email address/screen name is a good way to play it safe if you're not sure how into you she is. but yeah, no harm in asking-- you'll probably regret it if you don't at least ask anyways.

What the hell is a date?
i don't know, i never really go on them. i just like hanging out with whoever i'm with, i think the term a "date" is vague and overused. just do something fun that you've both agreed on (maybe in a public place like a park if it's the first time you guys are alone).
