There are a few websites that charge for this metabolic type test (usually comprehensive) - I know that Mercola website as a free sample test as well a more thorough one which you have to pay.
Dr Kelleys site has a link for his test book which you can order.
I bought Dr Wolcott's book and it has a 65 question to determine your type (I was not impressed with the book at all).

Nutrispec has a comprehensive way of determining several imbalances but its geared up to doctors. (I do take the some nutrispec supplements to correct my imbalances)

Once you have a general idea of your metabolic type (based on some of the info you gave you are most likely a fast oxidizer), it is wise to visit Dr Weston Price's website, because is provides priceless information on some diet myths and general guidelines to live healthy life.
