I have had the same problem since i was in my early 20's, I'm 37 and I was just diagnosed with Intertestial Cystitis...I had a cyscoscopy perform. The symptoms are exactly like you are describing. Even any type of sexual encounter is painful sometimes....Right now I'm taking elmiron (2 week)...The only side effect to this medication is loss of hair...The pain comes and goes and I even had to change my eating habits....Please have an urologist look into your symptoms...
