I'm not sure exactly how the heath care system works in the U.S. When you go to the doctor (general physician) do you pay cash upfront? If you need to go to a physical therapist or something, do you pay a large fee?

We pay a monthly fee in Canada and that entitles us to visit the doctor whenever we need to. I was under the impression that Americans had to pay quite a bit more out of their own pocket...
