A case report is usually because the occurrence is rare - your reference says unique. I would have to read the paper, but as the aphorism goes "never say never". You need to provide a reference. The explanation may be that the fistula was not found, that's all. Another explanation is that it was a normal sized gallstone blocking an abnormal bowel, so that paper would only apply to those with Crohn's disease.
That also doesn't mean that large Gallstones go through the bile ducts usually. You can't use such rarities to explain the liver flush.
