When you see stones in the bile duct causing obstructive jaundice they are small. If even these small stones get blocked, why wouldn't larger stones?
Also another sign of the fistula is the aerobilia often found, as air goes from the bowel to the bile ducts via the gallbladder. Aerobilia alos occurs after ERCP when a papillotomy.
