I will add that I had this done, and my dentist ordered it herself. You are much better off getting a dentist who uses this as an integral part of his/her practice than trying to do it yourself. The thin notebook I received back is loaded with much more information than I want to interpret solo.

I had the blood drawn at my local hospital. I don't know if this is unusual, but I wasn't even charged for it.
