If there's no primary cause like coming into contact with a toxic agent, which it sounds like must be the cause because of how they're isolated on just the one shoulder, then it could be a healing crisis from something that happened to you in the past like a time when you had poison oak, or too many toxins in the lymphs.

To deal with the sores you can use clay mixed with colloidial silver to draw out the toxins. Internally you can step up bowel management (get a colonic), drink more water. You may want to do a modified detox bath - no cayenne pepper, the rest should be okay.
