You are 100% right-on! In the ER my husband and I told the doctors and nurses several times not to give me Demerol because I am severely allergic to it. Well, they gave me Demerol and I reacted to it just as I said. The records do not show we told them anything, even though my allergy information is on all my previous records that they were referring to. To add insult to injury, they wrote about my allergic reaction saying I'm a drama queen.

The first three times I went to a new rheumatologist I couldn't walk and was in a wheelchair. On subsequent visits I was able to use a cane. At all visits I complained of back and leg pain. At my LAST visit with him I was again in a wheelchair. He looked at his notes and said, I haven't seen you in a wheelchair before. Why are you using a wheelchair?

After my cousin's surgery, she rang the buzzer repeatedly for a bed pan, and no one came. Her bladder finally let loose. Years later, a new doctor asked her how long has she had her bed wetting problem?!!!

The list of outright lies and exagerations in my medical records goes on and on.
I HAVE given up on mainstream medical doctors.
