A new form for the Medical vaccination Exemption is now required by Proclamation of the Department of Health !! Old exemptions will NOT be grandfathered in. Childcare, schools, etc. will demand those and they cannnot accept old form certificates after July 1, 2019 (some are already demanding them). (This also applies to Adult Certificates.)

This will be a massive undertaking for doctors. We will be working day and night, and if we do not handle this now we will be overwhelmed.
