You do not need a surgical scalpel, you need a needle, alcohol or iodine.

Take a needle, sterilize it with some strong wodka or pure alcohol or iodine, and just make a small enough hole to get the liquid out.

Sterilize both the needle and the skin before making the puncture.

Needle can also be sterilized with fire, but wait for it too cool down before using it, or it can burn your skin.
