http://sci-hub.bz/ has now been blocked by disabling it's DNS. See the article on Theregister for details:

There are still ways to access it and other URLS, some of which have also been blocked

You can also use the IP addresses to access them to bypass DNS:
80.82.77.83
80.82.77.84

I do not have an IP address for the .tw domain ATM. The two IP address that I came up with do not allow access due to cloudflare prevention.

There might be some DNS servers which have not implemented this but even my no censorship servers seem to have it blocked.
