I don't have the answers to many of your questions. I hope others more experienced will offer advice. But if you want to stay all natural, maybe try adding diatomaceous earth -- work up to 1 tablespoon in a glass of water twice daily. The other thing, if you're willing to try it, would be ivermectin.
