I had every symptom you listed. I just had my mirena removed yesterday. My pain was to the point were I felt I had a knife stabbing me, through my stomach/uterus to the right side. I've had my mirena 6 1/2 years, with weight gain (~30lbs), headaches, anxiety/depression, insomnia, irritability...you name it. The minute the IUD was removed, I felt the irritation go away. I could stand straight again, sit in a chair, and function. The last two weeks, it had shifted somehow and irritated the right side of my uterus. I thought I was going to die! Moral of the story: listen to your gut (no pun intended). If you feel that there is something wrong, chances are there is. This is a foreign body inside YOUR body. Realistically it should help you but if you can't go about your day in a normal way, get it out ;)

And the hot flashes for me were night sweats. I would wake up and be drenched! It was terrible!!!
